Please print your emailed confirmation page and bring in to NorthPointe Wellness along with your completed BHS Payroll Authorization Form.  You will be asked to complete the Health History Questionnaire (HHQ) and will be issued a temporary membership card.  You will receive your permanent scan card following the completion of your health assessment.  A fitness team member will be in contact with you upon completion of your HHQ to schedule your fitness assessment.
